The Risk Mitigation Policy represents the City’s approach to preventing, mitigating, and minimizing displacement.

Job Summary
Under general direction, the Community Health Worker will work under the supervision of the Healthy Neighborhoods Program Coordinator and Healthy Neighborhoods Team Leader on grant deliverables related to childhood obesity prevention strategies. The Community Health Worker will provide health education and community engagement leadership in a community setting in San Antonio. The Community Health Worker will be responsible for working collaboratively with other grassroots organizers and community leaders, providing health education and community organizing presentations to groups, and supporting local health initiatives.

The ideal candidate will have a passion for improving health outcomes in low income communities of color, the ability to promote a positive work environment through collaboration and teamwork, and will have experience in community organizing and health promotion.  Exercises no supervision.

This position is a “grant funded” position and has no disciplinary appeal rights to the Municipal Civil Service Commission. This position is likewise subject at-will employment status and can be terminated at any time for any reason.

Essential Job Functions
  • Employs strategic community building and organizing techniques to engage key internal and community stakeholders around health issues and health equity, including grassroots community based organizations, health centers, schools, community leaders, and community members.
  • Identifies or connects members of high-risk or otherwise targeted groups, such as members of minority populations, low-income populations, or people experiencing homelessness.
  • Help community members understand health information and connect to health resources in a culturally appropriate way.
  • Provides feedback to public health providers about how to make their services more accessible or culturally appropriate.
  • Refers community members to needed health and social services.
  • Provides health education or otherwise disseminate medical or dental health information to school groups, community groups, or targeted families or individuals, in a manner consistent with cultural norms.
  • Collects information from individuals about the general health of community members.
  • Works with individuals and communities to identify priority health needs.
  • Maintains updated community engagement records with plans, notes, or related information.
  • Advocates for individual or community health needs with government agencies or health service providers.
  • Attends community meetings or health fairs to understand community issues and build relationships with community members.
  • Follows up with community members by phone or in person about health referrals.
  • Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community.
  • Performs related duties and fulfill responsibilities as required.
Job Requirements
  • High School Diploma or GED equivalent (recognized by the Texas Education Agency or a regional accrediting agency).
  • Two (2) years of experience working as a Community Health Worker (CHW), health educator, community outreach worker, or community organizer.
  • This position requires a valid Class “C” Texas Driver’s License OR a valid driver’s license from another state with the ability to obtain a valid Class “C” Texas Driver’s License within 30 days of becoming a Texas resident.  Use of your own personal vehicle may at times be required and proof of Liability Insurance will be required.
  • Any applicant selected for this position must have a background check completed with results satisfactory to the City of San Antonio and successfully complete the San Antonio’s Cash-Handling Certification Training.

PREFERRED QUALIFICATIONS:

  • Bilingual in English and Spanish.
  • Community Health Worker Certification.
  • Experience working with vulnerable populations.
  • Experience with community organizing.
  • Experience providing health education to a diverse range of group types.
  • Experience providing physical activity or fitness classes.

One year of full-time experience is defined as 30 or more hours worked per week for 12 months.  One year of higher education is defined as 30 credit hours completed at an accredited college or university.
  • Applicants selected for employment with the City of San Antonio in this position must receive satisfactory results from pre-employment drug testing and background checks.  If required for the position, a physical, motor vehicle record evaluation, and additional background checks may be conducted.

PHYSICAL REQUIREMENTS:

  • Physical requirements include frequent lifting/carrying up to 5 pounds and occasional lifting/carrying up to 25 pounds; visual acuity, speech and hearing; hand and eye coordination and manual dexterity necessary to operate a computer keyboard and basic office equipment. Subject to sitting, standing, reaching, and walking to perform the essential functions. Working conditions are primarily in a community-based environment and an office environment.

  • Knowledge of programs and services provided by governmental and non- governmental organizations that work to improve the health of the community.
  • Knowledge of basic public health issues, methods and principles.
  • Knowledge of social skills demonstrative of empathy and support.

  • Skill in utilizing a personal computer and associated software programs.
  • Ability to facilitate meetings in either community or home settings.
  • Ability to successfully manage competing demands.
  • Ability to work within a team environment and work collaboratively to accomplish common goals and objectives.
  • Ability to share information to professionals and the public using a variety of approaches.
  • Ability to communicate clearly and effectively, both verbally and in writing.
  • Ability to plan, organize, and implement educational programs.

Statewide Birth-Five Needs Assessment Survey

The Texas Early Learning Council (TELC) is conducting a statewide survey to better understand the needs and challenges of those accessing, providing, and supporting early childhood programs and services in Texas.
The survey is open from Monday, March 25, 2019 to Friday, April 5, 2019 and should take no more than 20 minutes to complete. Questions intended for families and early childhood program and service providers are available in both English and Spanish. Primary Purpose – Provides all navigation assistance for 1115 Waiver program.

 

Major Duties and Responsibilities

Complete real time patient assistance in the hospital, Emergency Department and office walk-ins.

With social resources and screenings, follow up appointments, post discharge plans and education.

Creates and executes readmission prevention plan for enrolled patients.

Promotes Nix Health services and providers when appropriate.

Maintains and updates Patient Navigation Database; utilizes daily reports to evaluate admissions and track frequent utilizers.

Problem solves issues, gathers data and provides information as necessary.

Participates in daily safety huddles and monthly staff meetings as necessary.

Participates in organizational process improvement and continued regulatory compliance for continued improvement in patient outcomes.

Maintains current knowledge in best practices, organizational policies and procedures in social determinants of health and community health.

Completes special projects and other related duties as assigned.

 

Minimum Requirements  Experience:

1. Cognitive skills as related to position.

2. Proven independent judgement with effective outcomes.

3. Strong attention to detail, proficient writing skills and strong communication skills.

4. Strong customer service with proven team building skills.

Texas Governor Greg Abbott issues Proclamation for April 8, 2019 as

“COMMUNITY HEALTH WORKER DAY IN TEXAS”

 

March 15, 2019, Houston, Texas. Texas Governor, Greg Abbott, issued a Proclamation recognizing April 8th 2019as “Community Health Worker Day in Texas”.  Hundreds of Promotores/Community Health Workers from across Texas will be in Austin celebrating this milestone beginning on April 7th with a 4-hour Advocacy Training and a visit to the state Capitol on April 8, 2019 to receive the recognition by both the Senate and the State Representatives.

In 1999, Texas became the first state in the nation to formally recognize Promotores and Community Health Workers (P/CHWs) for their contributions to keep Texans healthy by addressing disparities among underserved and underrepresented vulnerable populations. The population at that time was 20,851,820, with 46% representing a diverse community. The prevalent health concerns were Cardiovascular Disease, Cancer and Communicable Diseases.

Today, 20 years later, the population is 29.10 million, with 55% representing a vast multicultural community and the most pressing health focuses are Medicaid, Opioids and Substance  Abuse, Adult Protective Services, Maternal care, Mental Health, and Vaccines.

“More than 4000 Promotores/Community Health Workers in Texas are trained, certified and equipped to effect positive change in the health and human services landscape”, stated Venus Ginés, who has been training Promotores/CHWs since 1997 in over 39 states and Puerto Rico. Some are working in community clinics, hospitals, and federally qualified health centers, while others have been instrumental in conducting health assessments and early detection screening, such as blood pressures, diabetes testing, HIV testing and vision screening.  During Hurricane Harvey, these trusted members of our community were out in full force rebuilding Houston and its residents into resiliency.

As we look to the future, Texas Promotores/Community Health Workers are fully prepared of playing a greater role in helping populations affected by the social determinates of health to address health disparities through various medians such as in person engagements, via text, social media, tele-health, or in group care settings. Expanding the role of Promotores and Community Health Workers has been the mission of Dia de la Mujer Latina, Inc., the nonprofit who launched the Annual P/CHW Conference since 2011.
