POSITION SUMMARY/RESPONSIBILITIES This position will serve as the link between HRSA RMOMS network sites in Uvalde, Del Rio and University Health System in order to maximize the use of existing health services deemed appropriate for high risk perinatal and postpartum patients. Assists and guides patients to ensure that patients are able to access medical appointments and participate in recommended care plans. Works closely with Perinatal Case Managers at network sites and the Maternal Fetal Medicine Clinical Coordinator at University Health System to link patients to resources, including, but not limited to Medicare applications, and assistance with medical transportation, when deemed necessary. Assists and supports with telehealth trainings for the RMOMS program. Additional activities will include patient tracking, data entry, and reporting.
