The Health Collaborative is hiring certified Community Health Workers to join our team! The Health Collaborative is seeking passionate individuals interested in helping people in Bexar County navigate and access services and resources.
The CHW position will support the Grow Healthy Together Pathways HUB program: a data driven, care coordination system made up of CHWs from organizations throughout the community, working together to empower individuals and families to improve their health outcomes.
