The ideal candidate will have a passion for improving health outcomes in low income communities of color, the ability to promote a positive work environment through collaboration and teamwork, and will have experience in community organizing and health promotion.  Exercises no supervision.

This position is a “grant funded” position and has no disciplinary appeal rights to the Municipal Civil Service Commission. This position is likewise subject at-will employment status and can be terminated at any time for any reason.

• Employs strategic community building and organizing techniques to engage key internal and community stakeholders around health issues and health equity, including grassroots community based organizations, health centers, schools, community leaders, and community members.

• Identifies or connects members of high-risk or otherwise targeted groups, such as members of minority populations, low-income populations, or people experiencing homelessness.

• Help community members understand health information and connect to health resources in a culturally appropriate way.

• Provides feedback to public health providers about how to make their services more accessible or culturally appropriate.

• Refers community members to needed health and social services.

• Provides health education or otherwise disseminate medical or dental health information to school groups, community groups, or targeted families or individuals, in a manner consistent with cultural norms.

• Collects information from individuals about the general health of community members.

• Works with individuals and communities to identify priority health needs.

• Maintains updated community engagement records with plans, notes, or related information.

• Advocates for individual or community health needs with government agencies or health service providers.

• Attends community meetings or health fairs to understand community issues and build relationships with community members.

• Follows up with community members by phone or in person about health referrals.

• Distributes flyers, brochures, or other informational or educational documents to inform members of a targeted community.

• Performs related duties and fulfill responsibilities as required.

• High School Diploma or GED equivalent (recognized by the Texas Education Agency or a regional accrediting agency).

• Two (2) years of experience working as a Community Health Worker (CHW), health educator, community outreach worker, or community organizer.

• This position requires a valid Class “C” Texas Driver’s License OR a valid driver’s license from another state with the ability to obtain a valid Class “C” Texas Driver’s License within 30 days of becoming a Texas resident.  Use of your own personal vehicle may at times be required and proof of Liability Insurance will be required.

• Any applicant selected for this position must have a background check completed with results satisfactory to the City of San Antonio and successfully complete the San Antonio’s Cash-Handling Certification Training.

PREFERRED QUALIFICATIONS:

• Bilingual in English and Spanish.

• Community Health Worker Certification.

• Experience working with vulnerable populations.

• Experience providing health education to a diverse range of group types.

• Experience providing physical activity or fitness classes.

• Unless otherwise stated, applicants are permitted to substitute two years of related full-time experience for one year of higher education or one year of related higher education for two years of experience in order to meet the minimum requirements of the job. One year of full-time experience is defined as 30 or more hours worked per week for 12 months.  One year of higher education is defined as 30 credit hours completed at an accredited college or university.

PHYSICAL REQUIREMENTS:

• Physical requirements include frequent lifting/carrying up to 5 pounds and occasional lifting/carrying up to 25 pounds; visual acuity, speech and hearing; hand and eye coordination and manual dexterity necessary to operate a computer keyboard and basic office equipment. Subject to sitting, standing, reaching, and walking to perform the essential functions. Working conditions are primarily in a community-based environment and an office environment.
