WellMed provides concierge – level medical care and service for seniors, delivered by physicians and clinic staff that understand and care about the patient’s health. WellMed’s proactive approach focuses on prevention and the complete coordination of care for patients. WellMed is now part of the Optum division under the greater UnitedHealth Group umbrella.
This position is grant funded and part of the WellMed Charitable Foundation. The goal of this job is to assist in protecting, maintaining and building trust between patients, their local community, and the clinical team by understanding patients’ needs and capacities; facilitating patients’ compliance with clinical treatments; supporting patients as they implement changes that improve their self-care; linking the patients to community based services to improve their living situation, and educating providers about the contextual social determinants of their patient’s health.
